The information you provide here will be tremendously helpful to me as I coordinate the program books, refreshments, and everything else that makes DMGS3 so special. Thanks for taking the time to be thorough and thoughtful in your responses. I’m so very appreciative!
(If you’re feeling confused, you may want to read through the DMGS3 Performer FAQs before returning to complete this form!)
(this may be a parent/guardian, if the student is a minor)

IMPORTANT!

Please note that DMGS3 2026 will be videorecorded in full, and that video will be made available to faculty at Shenandoah Conservatory as David’s final assignment toward the Master of Music degree.  By agreeing to participate in DMGS3 2026, you authorize David to share the audio/video recording of your participation in this way.

Are you interested in performing in the 2026 Spring Studio Showcase?
Share either a list of “bullet points” or roughly 25–75 words of prose describing your work/school, family members/pets, hobbies, aspirations, etc. All submissions are subject to editing for space and other considerations. PLEASE NOTE that overly brief (or non-existent) responses to this question will be considered permission for David to write a biography on your behalf that may or may not have any basis in actual documented reality, and will be crafted with an eye toward maximum humorous effect.
Would you be willing to stay after the Showcase to record a brief video “commercial” for David’s teaching studio?
Samples of past video interviews are available at https://davidmglasgow.com/teaching/testimonials . If you express interest, I’ll send a list of possible questions for you to consider as you think about what to say when the cameras are rolling.

Household Information

(If more than one member of your household plans to perform at DMGS3 2025, I ask that only ONE of you include the following information in your Planning Form.)
Has anyone else in your household already submitted a copy of this Planning Form?
This count helps us to estimate quantities for refreshments, etc. Don’t worry about exact counts—all are welcome, even if they haven’t “RSVP’ed”!

Be a DMGS3 Patron!

The Spring Studio Showcase is my absolute favorite event of the year, and I consider the experience of participating in this event to be invaluable, so I want all of my students to feel welcome to participate—and their loved ones welcome to attend—regardless of their financial situation.  That’s why I include DMGS3 participation in all subscription plans, and for 2026 I’m even waiving the fee for PPL students, to maximize participation for my Master’s submissions.

That said, the professional videographers, audio technician, and instrumentalists who support this event, along with the costs of using the space, printing programs, etc., amount to more than $3000 each year.  (And many who have attended past DMGS3 showcases have enthusiastically shared that they feel the event is worth paying for!)  If your family has room in your budget to provide support for this project, contributions of any size are appreciated and help to minimize the “dip” in my bank account that happens each time I hold the Showcase.

There will be an opportunity to make cash and check contributions at the event, but if you’d like to help me express my gratitude to the many professionals who help make this event amazing by supporting the event at a patron level, please complete the following section.  I’ll be delighted to include your name in the program, if you’ll let me!
